DPH Privacy Policy
We understand that medical information about you and your health is personal. We are committed to protecting medical information about you. We create a record of care and services you receive from us. We need this record to provide you with quality care and to comply with certain laws. This Notice applies to all records that contain your personally identifiable health information. The Notice describes the privacy practices that the DPH and all of our employees and other personnel are required to follow in handling your protected health information. We are required by law to:
- Keep your medical information, also known as “protected health information” or PHI, private;
- Give you this Notice of our legal duties and privacy practices with respect to your PHI;
- Follow the terms of the Notice that is currently in effect.
- The DPH agrees to abide by the terms of the Notice.
