The Refund of Contribution
application posted here is provided so that you may preview a copy of
the forms that are required in order to withdraw pension contributions
upon termination of employment. These sample forms are not official
forms of the City Employees’ Pension Fund. The forms found on this
site are “specimen” forms, and are for informational purposes only.
The application should be completed in the Pension Administration
Office, and a pension administration staff member witnesses your
signature. Please contact the Pension Administration Office to
schedule an appointment to complete an application, or to make other
arrangements if you are not able to come into the office. |
