Naming of Transfer Centers Form

Below you can download the forms necessary to apply for the Sun Metro IDs for reduced fare. Please mail the completed application(s) to the mailing address to the left or bring them to the Bert Williams Downtown Santa Fe Transfer Center. In order to receive a Sun Metro ID you must come to the Bert Williams Downtown Santa Fe Transfer Center to have your picture taken. Groups of 20 or more may contact us to have our mobile ID representatives come to your location.

1. All applicants must complete the following form:
   (SM B) Application for Reduced Fare ID (fill-in and print-out)
   (SM B) Application for Reduced Fare ID (print-out and fill-in)
2. Disabled applicants must complete form SM B and form SM A:
   (SM A) Application for Disabled Certification (fill-in and print-out)
   (SM A) Application for Disabled Certification (print-out and fill-in)
3. LIFT applicants must complete form SM B and the following form:
   Sun Metro LIFT Application (fill-in and print-out)
   Sun Metro LIFT Application (print-out and fill-in)
   LIFT applicants must also have a doctor complete the following form:
   LIFT Doctor Questionnaire (fill-in and print-out)
   LIFT Doctor Questionnaire (print-out and fill-in)

Note: Due to a change in policy, it is now necessary to present a Sun Metro ID in order to purchase discount passes.

Naming of Transfer Centers Form   Sun Metro Citizen’s Advisory Committee is currently accepting nominations for the naming of the Northeast Transfer Center.   The deadline to submit nominations is July 1, 2010.   In order to nominate an individual, the nomination form must be submitted to Sun Metro’s administrative offices, 700-A San Francisco, along with 50 signatures of persons living within Sun Metro’s service area and a full biography of the individual.      Click [here]  to download and print the policy for naming a transfer center.      Click [here]  to download and print the form for naming a transfer center.
