Below you can download the forms necessary to apply for the Sun Metro IDs for reduced fare.  Please mail the completed application(s) to the mailing address to the left or bring them to the Union Plaza Transit Terminal. In order to receive a Sun Metro ID you must come to the Union Plaza Transit Terminal to have your picture taken. Groups of 20 or more may contact us to have our mobile ID representatives come to your location.

1. All applicants must complete the following form:
   (SM B) Application for Reduced Fare ID (fill-in and print-out) 1.79MB
   (SM B) Application for Reduced Fare ID (print-out and fill-in) 96KB
2. Disabled applicants must complete form SM B and form SM A:
   (SM A) Application for Disabled Certification (fill-in and print-out) 0.98MB
   (SM A) Application for Disabled Certification (print-out and fill-in) 183KB
3. LIFT applicants must complete form SM B and the following form:
   Sun Metro LIFT Application (fill-in and print-out) 0.99MB
   Sun Metro LIFT Application (print-out and fill-in) 76KB
   LIFT applicants must also have a doctor complete the following form:
   LIFT Doctor Questionnaire (print-out and fill-in) 62.1 KB
   
   LIFT Doctor Questionnaire (fill-in and print-out) 92.2 KB
